Name the exact field you are disputing and the value currently reported. 3. State the value your own dated document shows, and name that document and its date. 4. Write one sentence saying the item is inaccurate or incomplete, and why. 5. State the specific correction you are requesting. 6. Attach the report page with the item circled, plus copies of your supporting documents. 7. List your enclosures, and confirm you are sending copies and keeping originals. 8. Keep a copy of the whole letter and everything you sent with it.
